This Notice of Privacy Practices is shared with clients, foster parents and/or caregivers and explains how Pathways may use or disclose information. Pathways is required to provide a notice of our privacy practices for the information we collect and keep about an individual. The Agency is required to follow the terms of the notice currently in effect according to the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
